Patient Financial Responsibility

  • Welcome and thank you for choosing Skin Care Doctors, P.A. and Vein Associates for your care. Please carefully review the following information and ask if you have any questions about our fees, policies, and/or your responsibilities.

    Registration/Check-in: At the time of registration, and periodically thereafter, you will be asked to complete a Registration Form to help keep personal and insurance information accurate. You will be asked to present a valid photo ID and current insurance card when you check in for each appointment. Any past due balance will be collected at the time of check-in. Patients under the age of 18 must have a parent or legal guardian in attendance at their first appointment.

    Insurance – Skin Care Doctors, P.A. and Vein Associates participate with most major health insurance plans. It is the patient’s responsibility to check with their insurance company to confirm our clinic and providers are in network. The patient is also responsible for knowing their insurance benefits as insurance plans, policies, and coverages vary. We will gladly file your medical visit to the insurance companies we participate with. Failure to provide accurate and up to date insurance information may result in the full balance due from the patient. We will not become involved in disputes between you and your insurance company regarding coverage or benefit criteria such as deductibles, co-pays, co-insurance, non-covered services, and coordination of benefits. You are responsible for all coinsurance, deductible, and copay amounts assessed by your insurance company(s). Co-payments are due at the time of service. Please note patients that are unable to provide proof of coverage or do not have health insurance will be required to pay for services the day of their appointment. We accept cash, personal checks, money orders, Visa, Discover, and MasterCard.

    Referrals: If the patient’s insurance plan requires a referral to be seen at our practice; please contact your primary care physician and have them send a referral to your insurance company prior to your visit with us. It is up to the patient to discern if a referral will be necessary.

    Preventative Care: Skin Care Doctors, P.A. and Vein Associates specialize in dermatology and venous care respectively therefore we do not bill out for Preventative Care. Preventative Care is provided by your Primary Care Provider or Specialty providers who render service that your Primary Care Provider cannot render (example: Colonoscopy or Mammogram).

    Cosmetics-HSA/HRA/Flex/Bene Plans: We do not accept Health Savings Account payments when paying for product or any cosmetic services. When scheduling a cosmetic appointment, a $100 deposit will be collected to reserve the appointment. This deposit is then applied to the amount charged for the cosmetic treatment with the balance due at the time services are rendered. A 24-hour cancellation notice is required and failure to do so will result in the loss of the $100 deposit. We can provide you with a receipt if you wish to submit for reimbursement on your own behalf.

    Returned Checks: : If a check is returned as unpaid by your bank, a $35 returned check charge will be applied and we will no longer accept checks as a form of payment.

    Collections: Please contact us if you have trouble paying your bill. Accounts with an outstanding balance over 60 days past due that have failed to make payment arrangements may be turned over to a collection agency. If your account is turned over to a collection agency, all non-emergent appointments will be cancelled until the collection account has been paid. Once paid, you may schedule appointments at our office, but your account will be on a cash-only basis for one year allowing time to reestablish credit worthiness. During this time, payment is required at the time of the appointment. As a courtesy, we will forward a claim to your insurance company(s). We will issue a refund to you if/when they pay on your claim.
