What will I have to pay at my appointment?

Copayments with Insurance: Patients will be required to pay their copayment prior to their appointment.

No Insurance: Patient's without insurance will be required to pay the self-pay pricing for their visit, and additional fees may incur for necessary procedures.

Cosmetics: Patient's having a cosmetic service will have to pay in full prior to the service.


What do I need to bring to my appointment?

Please make sure you have your photo ID, a valid insurance card, and method of payment (if applicable).


What can I expect for my surgical appointment?

Surgical appointments on surgery days can be a little longer than normal appointments.

Our receptionist will call you in the morning to see how far you live from our office. Depending on how the surgical day is running, we might ask you to come in later than your appointment time. We will try our best to accommodate your time and reduce your time in the waiting room.

Do not panic if you time has been moved to a later time, you will still be called in and seen for your surgical appointment that day.

Please plan to bring a snack, beverage, book, etc to your surgical appointment, as certain procedures require you to wait for results in our waiting room afterwards.


Can I download my patient forms?

Yes. The forms you need may be viewed / downloaded here. Please note that, like all medical practices, we will be shifting away from paper forms and moving to “Patient Portals.” However for now, please go to our Patient Forms page here.
